
Lutheran Family Services of Virginia's currently offers Geriatric Care Assessment in the Richmond area and I&R statewide. Over the next year, we will be developing programs that help older adults age in place in their homes and communities, caregiver support, and collaboration with a number of community and faith-based organizations to assist older adults in managing life changes.
What is a Comprehensive Geriatric Assessment? This important tool looks at the psychological, emotional, medical and physical situation of the older person; family, social and community supports; legal and financial status; and environmental safety. An assessment is helpful when an adult needs home care services, in-home care assistance, assistance with relocation, placement in a nursing home or assisted living facility, or help with other age-related issues.
What is I&R (Information & Referral)? Information and referral services help you care for an older adult (such as a parent, spouse, other relative or partner) by connecting you and your loved one with community resources, such as home health agencies, elder law specialists, and housing options, even from a distance! When you call Lutheran Family Services of Virginia, you will talk to an I&R specialist who will help you assess your needs in a confidential manner so that you may make informed decisions about possible solutions.
